Hospice Aide

The Hospice Aide provides personal care for patients in their place of residence (homes, skilled nursing facilities, residential care facilities) and assists with their activities of daily living. Qualified applicants must possess a current CNA license and have or be willing to obtain HHA certification. Candidates must also have a valid driver’s license, an excellent driving record, proof of acceptable automobile insurance coverage, and reliable transportation.  This is a benefited position.  Schedule: 4 days (32 hours/week)

 

Director of Quality

The Director of Quality provides leadership and oversight to the agency in all matters pertaining to compliance and quality.  The Director is a member of the Management Team and participates in agency-wide program development/coordination.  The position also serves as Hospice of Humboldt’s Compliance Officer, Risk Manager and Privacy Officer, and provides oversight for Medical Staff credentialing and the agency’s infection prevention plan.  Qualified applicants must have a minimum of two years’ experience in health/social services data management and/or healthcare administration.   Sign-on Bonus / Relocation Package offered!

 

RN – Clinical Manager

As a key member of the nursing leadership team, the Clinical Manager supervises clinical staff, coordinates the interdisciplinary group, and ensures compliance with all regulations.  This position requires excellent critical thinking, clinical judgment, and mentoring/teaching skills.  Candidates must have a current California Registered Nurse license, be a graduate of an accredited nursing program, and have two years’ professional nursing experience.    Schedule: 4 day/week or 5/day week options; Sign-on Bonus offered!

 

RN – Float Nurse

The Float Nurse provides care to hospice patients in collaboration with an interdisciplinary team and under physician’s orders.  The Float Nurse functions in various hospice nursing capacities to cover absences of regularly scheduled staff.   This benefited position requires the ability to work a variety of shifts and schedules, including nights and weekends.  Candidates must have a current California Registered Nurse license and be a graduate of an accredited nursing program.  Schedule: 30-37 hours/week;  Sign-on Bonus offered!

 

RN – Case Manager

The Case Manager is responsible for assessing needs and coordinating care, as well as providing direct nursing care for patients in their place of residence.   This position is an integral part of the hospice interdisciplinary care team.  Ideal candidates will possess excellent teamwork and clinical judgement skills.  Candidates must have a current California Registered Nurse license and be a graduate of an accredited nursing program.  Schedule: 30-37 hours/week;  Sign-on Bonus offered!

 

RN – Visit Nurse

The Visit Nurse provides direct nursing care for patients in their place of residence in collaboration with an interdisciplinary team and under physician’s orders.   This position requires the ability to adapt to schedule changes and high priority patient needs.  Candidates must have a current California Registered Nurse license and be a graduate of an accredited nursing program.  Schedule: 30-37 hours/week and Per Diem options available;  Sign-on Bonus offered!

 

RN – Hospice House Nurse

The Hospice House RN provides nursing care for patients living in the new Ida Emmerson Hospice House in-patient facility.   The Nurse actively participates in the coordination of all aspects of patient care.  This ideal applicant will enjoy working in a team-environment and have a passion for Hospice’s mission.  Candidates must have a current California Registered Nurse license and be a graduate of an accredited nursing program.  Schedule: DAYS, 24-hours/week (2 shifts);  Sign-on Bonus offered!

 

Hospice House Aide

The Hospice House Aide provides personal care for patients and assists with their activities of daily living.  Working 12-hour shifts in the new Ida Emmerson Hospice House in-patient facility, this position will also assist with housekeeping and dietary/food service responsibilities.  Qualified applicants must possess a current CNA license and have or be willing to obtain HHA certification.  Schedule: 36 hours/week (3 shifts)
