Director of Social Services

The Director of Social Services provides leadership for the Social Services Department and is responsible for ensuring quality social work, chaplaincy, grief support, and volunteer services are provided to Hospice patients. This position is a member of the Management Team and participates in agency management, program development and qaulity assurance activities. Qualified applicants must have a Master’s degree in Social Work (MSW), two years of supervisory experience and three years of clinical experience in a health care/social services setting.

 

Community Outreach Liaison

The Community Outreach Liaison represents Hospice of Humboldt to referral sources, such as physicians, discharge planners, social workers, facility staff, and healthcare decision makers.  The position is responsible for developing and implementing marketing strategies to promote Hospice of Humboldt to the community, producing marketing collateral and maintaining the agency’s digital properties. Qualified candidates will have two years of marketing/sales experience, familiarity with the healthcare field, and excellent relationship-building skills.  This is a full-time, benefited position.

 

RN – Hospice House Nurse

The Hospice House RN provides nursing care for patients living in the new Ida Emmerson Hospice House in-patient facility.   The Nurse actively participates in the coordination of all aspects of patient care.  This ideal applicant will enjoy working in a team-environment and have a passion for Hospice’s mission.  Candidates must have a current California Registered Nurse license and be a graduate of an accredited nursing program.  Schedule: 12-hour shifts – DAYS;  Sign-on Bonus offered!

 

Hospice House Support Assistant

The Hospice House Support Assistant prepares food for patients residing in the Ida Emmerson Hospice House (IEHH) and maintains the cleanliness of the facility.  The position also provides general support to the IEHH patient care staff, including answering phones, greeting visitors and sitting with patients.  Candidates must have a valid driver’s license, an excellent driving record, proof of acceptable automobile insurance coverage, and reliable transportation.  This is a benefited position.  Schedule: Part-time; 8-19 hours/week

 

Hospice Aide

The Hospice Aide provides personal care for patients in their place of residence (homes, skilled nursing facilities, residential care facilities) and assists with their activities of daily living. Qualified applicants must possess a current CNA license and have or be willing to obtain HHA certification. Candidates must also have a valid driver’s license, an excellent driving record, proof of acceptable automobile insurance coverage, and reliable transportation.  This is a benefited position.  Schedule: 4 days (32 hours/week)

 

RN – Float Nurse

The Float Nurse provides care to hospice patients in collaboration with an interdisciplinary team and under physician’s orders.  The Float Nurse functions in various hospice nursing capacities to cover absences of regularly scheduled staff.   This benefited position requires the ability to work a variety of shifts and schedules, including nights and weekends.  Candidates must have a current California Registered Nurse license and be a graduate of an accredited nursing program.  Schedule: 30-37 hours/week;  Sign-on Bonus offered!

 

RN – Per Diem Nurse

The Per Diem Nurse works on an as-needed basis to provide direct nursing care for hospice patients in their place of residence — homes, residential care and skilled nursing facilities, and the Ida Emmerson Hospice House.   This position functions in various hospice nursing capacities and must have the ability to adapt to schedule changes and high priority patient needs.  Qualified candidates will have a current California Registered Nurse license and be a graduate of an accredited nursing program.   Sign-on Bonus offered!

 

Social Worker – Per Diem

The Per Diem Social Worker assesses and responds to the psycho-social, practical, financial, and legal concerns of Hospice patients and family members.  Social work services are provided to Hospice patients and family member in their place of residence, including homes, residential care and skilled nursing facilities, and the Ida Emmerson Hospice House.   This position works on an as-needed basis and must have the ability to adapt to schedule changes and high priority patient needs.  Qualified candidates must have a master degree in Social Work (MSW) and two years of social work experience in the health care field.
