The Director of Quality provides leadership and oversight to the agency in all matters pertaining to compliance and quality. The Director is a member of the Management Team and participates in agency-wide program development/coordination. The position also serves as Hospice of Humboldt’s Compliance Officer, Risk Manager and Privacy Officer, and provides oversight for Medical Staff credentialing and the agency’s infection prevention plan. Qualified applicants must have a minimum of two years’ experience in health/social services data management and/or healthcare administration.
The Clinical Nurse Educator develops educational programs and coordinates clinical education for nursing staff in alignment with the organization’s standards of care, competency requirements regulatory standards, and promotes an environment of quality care for patients. The Clinical Nurse Educator also provides community-based clinical training and outreach on end of life and Hospice-specific clinical issues. Qualified applicants must have a current California Registered Nurse license, be a graduate of an accredited nursing program and have at least three years of nursing experience, including at least one year of hospice or palliative care experience. Candidates must also have or be willing to obtain CHPN certification. Schedule: 30-37 hours/week
The Float Nurse provides care to hospice patients in collaboration with an interdisciplinary team and under physician’s orders. The Float Nurse functions in various hospice nursing capacities to cover absences of regularly scheduled staff. This benefitted position requires the ability to work a variety of shifts and schedules, including nights and weekends. Candidates must have a current California Registered Nurse license and be a graduate of an accredited nursing program. Schedule: 30-36 hours/week
The Visit Nurse provides direct nursing care for patients in their place of residence in collaboration with an interdisciplinary team and under physician’s orders. The per diem position does not work a regular schedule and is assigned shifts based on availability and need. This position requires the ability to adapt to schedule changes and high priority patient needs. Candidates must have a current California Registered Nurse license and be a graduate of an accredited nursing program.
The Hospice Aide provides personal care for patients in their place of residence (homes, skilled nursing facilities, residential care facilities) and assists with their activities of daily living. Qualified applicants must possess a current CNA license and have or be willing to obtain HHA certification. Candidates must also have a current California driver’s license, an excellent driving record, proof of acceptable automobile insurance coverage, and reliable transportation. This is a benefitted position. Schedule: 4 days (32 hours)/week
