The Director of Quality provides leadership and oversight to the agency in all matters pertaining to compliance and quality. The Director is a member of the Management Team and participates in agency-wide program development/coordination. The position also serves as Hospice of Humboldt’s Compliance Officer, Risk Manager and Privacy Officer, and provides oversight for Medical Staff credentialing and the agency’s infection prevention plan. Qualified applicants must have a minimum of two years’ experience in health/social services data management and/or healthcare administration.
The Chaplain provides spiritual care to hospice patients and their families/caregivers and assists them with rituals and ceremonies. This per diem position works an intermittent schedule to cover shifts as needed. Candidates must have a Master’s degree in theology, pastoral counseling, or a related field from a seminary or equivalent educational institution. Schedule: Variable
The Case Manager is responsible for identifying needs and coordinating care provided to Hospice patients and their family/caregivers. This benefited position is best-suited for applicants who enjoy working in a team environment and have a passion for Hospice’s mission. Candidates must have a current California Registered Nurse license and be a graduate of an accredited nursing program. Schedule: 30-37 hours/week
The Visit Nurse provides direct nursing care for patients in their place of residence in collaboration with an interdisciplinary team and under physician’s orders. This position requires the ability to adapt to schedule changes and high priority patient needs. Candidates must have a current California Registered Nurse license and be a graduate of an accredited nursing program. Multiple positions available — 30-37 hour/week, benefited positions and Per Diem positions.
The Hospice House RN provides nursing care for patients living in the new Ida Emmerson Hospice House in-patient facility. This part-time, benefited night shift position actively participates in the coordination of all aspects of patient care. Candidates must have a current California Registered Nurse license and be a graduate of an accredited nursing program. Schedule: 24 hours/week
POSITION HIGHLIGHTS: Night shift differential!
The Hospice Aide provides personal care for patients in their place of residence (homes, skilled nursing facilities, residential care facilities) and assists with their activities of daily living. Qualified applicants must possess a current CNA license and have or be willing to obtain HHA certification. Candidates must also have a current California driver’s license, an excellent driving record, proof of acceptable automobile insurance coverage, and reliable transportation. This is a benefited position. Schedule: 4 days (32 hours)/week
